Concierge medicine is a membership-based relationship with a physician practice. Members pay a membership fee, and in return the practice limits the number of people it cares for, so that each member can have more time, more direct access and more continuity than a conventional appointment usually allows. Practices differ, so the most useful thing anyone considering membership can do is understand clearly what is included, and what is not.

What concierge membership usually includes

A continuing physician relationship. The centre of the model is a physician who knows your history, your current treatment and your priorities, and who stays with you over time rather than meeting you fresh at each visit.

Time and access. Longer conversations, easier scheduling and a direct route to the practice when a question comes up. Ask each practice exactly how access works, including response expectations and what happens outside office hours.

Care coordination. Helping you make sense of recommendations from different specialists, gathering relevant records, and clarifying the next appropriate step. Coordination is often where members notice the biggest difference.

Prevention and planning. A structured look at risk factors, screening decisions and long-term goals, rather than only the concern of the day.

Support around travel. For members who travel, planning ahead: discussing health needs before departure and how to reach appropriate care while away.

What it does not replace

Health insurance. A membership fee is not insurance. Hospital care, specialist care, imaging, surgery and many tests are still billed separately, and coverage still matters. Ask each practice how its membership relates to insurance before you join.

Emergency services. Concierge access does not replace 911 or an emergency department. Chest pain, signs of stroke, severe shortness of breath or any other emergency need emergency care immediately.

Your specialists. Cardiologists, surgeons, oncologists and other specialists remain part of your care. A concierge relationship works alongside them, helping the pieces connect rather than taking their place.

Your own judgement. Membership gives you more time and context for decisions; the decisions remain yours.

Questions to ask any practice

What exactly does the membership fee cover, and what is billed separately? How do I reach the physician, and how quickly should I expect a reply? How does the practice coordinate with my current doctors and specialists? What happens when I travel? And how are records and sensitive information shared securely?

Clear answers to those questions are the best indication of whether a concierge relationship will fit your life and your health needs.
